Report

A document leadership can work from.

Purpose

Why the Report stage exists

More than an audit. A list of problems is not a deliverable. The report exists to give leadership a usable path forward, with each finding attached to a recommendation, an owner and a date.

What you can expect

How this stage runs

  • An executive summary written for board and funder audiences
  • Findings ranked by risk with the reasoning shown
  • A corrective-action tracker with owners and target dates
  • Thirty-day improvement priorities so the work has a starting point
  • Training and policy recommendations tied directly to specific findings
Our role

What our nursing team does

  • Write in language a non-clinical reader can act on
  • Connect related findings so leadership sees the pattern rather than a scattered list
  • Recommend the smallest effective change wherever one exists
  • Provide multi-site trend reporting where more than one location is in scope

What we need from you

  • Field observations and documentation sampling from the review
  • Leadership context added during the debrief
  • Any clarifying records provided after the review day

What you receive

  • The written report and executive summary
  • A corrective-action tracker, available in the client portal where contracted
  • A recommended follow-up and reassessment plan
Moving forward

How this connects to the next stage

The corrective-action tracker becomes the working document for follow-up. It is the bridge between the review and real change.
